== Copper Canyon Ultra Marathon == In 2003, True decided to organize a race for the Tarahumara natives that would help them preserve their culture and running heritage. The first Copper Canyon Ultra Marathon was held on March 23, 2003; though turnout was small, it became an annual tradition. In 2006, True had the idea of inviting American ultrarunners to compete with the Tarahumara. After initially reaching out on the internet, he wrote an article in Men's Health, elucidating many of the lessons he internalized from the Tarahumara people. He also contacted Christopher McDougall, a writer for Men's Health, who would later write the book Born to Run. The 2012 event took place on March 12 and was the largest ever with hundreds of participants. Most were local Tarahumara, known for their running endurance, both as a general means of travel and while participating in ceremonial, team-based, long distance running events, most famously the Rarajipari, or ball game. In addition to prize money for the top ten finishers, the event awards seed corn vouchers to all runners who complete the distance. ==== Heart failure ==== Heart failure can be characterized by a loss of β adrenergic receptor (βAR) signalling in heart cells. When the βAR is stimulated by catecholamines such as adrenaline and noradrenaline, there is normally an increase in the contractility of the heart. However, in heart failure there are sustained and elevated levels of catecholamines which result in chronic desensitization of the βAR receptor. This leads to a decrease in the strength of heart contractions. Some research suggests that this chronic desensitization is due to the over activation of a kinase, G protein-coupled receptor kinase 2 (GRK2), which phosphorylates and deactivates certain G protein coupled receptors . When the G protein coupled receptor is activated, the Gβγ subunit recruits GRK2 which then phosphorylates and desensitizes GPCRs like the βAR. Preventing the interaction of the βγ subunit with GRK2 has therefore been studied as a potential target for increasing heart contractile function. The developed molecule GRK2ct is a protein inhibitor which inhibits the signalling properties of Gβγ subunit but does not interfere with alpha subunit signalling. The over expression of GRK2ct has been shown to significantly rescue cardiac function in murine models of heart failure by blocking Gβγ subunit signalling. == Clinical significance == Band neutrophils are an intermediary step prior to the complete maturation of segmented neutrophils. Polymorphonuclear neutrophils are initially released from the bone marrow as band cells. As the immature neutrophils become activated or exposed to pathogens, their nucleus will take on a segmented appearance. An increase in the number of these immature neutrophils in circulation can be indicative of an infection for which they are being called to fight against, or some inflammatory process. The increase of band cells in the circulation is called bandemia and is a "left shift" process. Blood reference ranges for neutrophilic band cells in adults are 3 to 5% of white blood cells, or up to 0.7 billion/L. An excess may sometimes be referred to as bandemia.
== Potential issues == While there is good evidence for the efficacy of opioid rotation as a treatment approach in general, there is less evidence for what particular opioid analgesics are most suitable, and in practice the choice of opioid drugs used depends on many factors such as patient characteristics, prescriber preferences and safety. One issue with opioid rotation is that an opioid therapy failure poorly predicts whether other opioids would be effective. In certain situations, multiple switches may be required before pain therapy is optimized. In addition, recent studies explore which opioid drugs are most effective in implementing in an opioid rotation, but have so far found no difference in efficacy between opioid drugs like methadone and fentanyl in cancer patients.
